* Sign on Bonus Eligible $20,000.

The Zone Medical Director provides direct medical care and population health management to patients served by Landmark Health. This role also provides clinical and operational oversight to a team of advanced level practitioners and the interdisciplinary team assigned to the zone. This role is a full-time position with the need to work a standard Landmark schedule.  

• Proactive ownership for market performance against key clinical and operational performance metrics:
• Provides clinical leadership, oversees quality and execution of clinical operations within a defined zone.
• Provides medical care to a panel of patients of the highest complexity
• Transitions high acuity patients back to longitudinal providers as the patient’s condition improves.
• Acts as a consultative physician focused on stabilizing the frailest and most vulnerable patients within the population.
• Provides on-call coverage to meet the needs of the market
• Provides real time supervision of Urgentivist Extenders (paramedics, LPN’s/LVN’s as needed)
• Supports advanced level practitioners with in-person or telephonic clinical consultations.
• Oversees the application of specific measurements of quality and ensure that all providers consistently meet thresholds.
• Supports implementation of centrally-led initiatives and develops market-led improvement initiatives.
• Provides mentorship to clinical team members and conducts regular 1:1s to proactively provide performance feedback.
• Recruits providers and pod leaders, ensuring the pursuit of talent with the appropriate mix of clinical skills and behavioral qualities to retain a high performing team.
• Participates in local market branding and recruiting activities or events.
• Assists Regional Medical Director with talent retention; holds performance discussions and provides growth opportunities for team members.
• Assists Regional Medical Director in developing and enhancing community relationships with hospitals, PCPs, community acute care, SNF, LTC and ALF facilities.
• Serve as an advocate for the team with community providers when escalation is necessary and ensure appropriate resolution.
• Develop a community-based Physician Advisory Board at the zone level and hold quarterly meetings.

• Current, valid, non-sanctioned, non-restricted license to practice medicine in the state
• Board Certified in Internal Medicine, Geriatrics sub-specialty or Family Practice [Board Eligible for up to one year if new grad]
• Ability to meet all Landmark Credentialing Requirements
• Extensive experience in providing medical care to geriatric patients
• Familiarity with Medicare Guidelines
• Familiarity with CPT and ICD-9 codes as related to billing practices.
• Malpractice profile meets Landmark’s risk guidelines.
• Minimum of four (4) years clinical experience, home-based care desired.
• Knowledge of clinical standards of care.
• Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful.


Certificates, Licenses, Registrations:
• Medical Doctorate or Doctorate of Osteopathy required
• Board Certified or Board Eligible in Internal Medicine, Geriatrics sub-specialty or Family Practice or Emergency Medicine
• Current updated DEA
Access to reliable transportation to conduct patient visits required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy

• Must be board certified at the time of hire or able to obtain board certification during the eligible timeframe (5 years following completion of residency or other training)
