Job Title: Clinical Nurse Profession: RN Specialty: Home Health Duration: 13 weeks Shift: Days Hours per Shift: 8 Experience: One year of nursing experience caring for acute and chronically ill patients. License: Licensed to practice as a Registered Nurse in the state of North Carolina. Certifications: N/A Must-Have: Nursing Diploma or ADN/ASN from an accredited school of Nursing. Description: Provides competent clinical nursing care consistent with professional standards. Reports to the Nurse Manager/Director and is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. Participates in identifying and meeting learning needs of self. Attends education programs based on identified learning needs. Uses patient education materials relevant to the patient population with assistance. Assesses readiness to learn of the patient, family, and caregivers. Evaluates the developmental level of patients and factors affecting their ability to learn. Identifies expected patient outcomes and seeks guidance as needed in the revision of the plan of care. Participates in discussions with members of the interdisciplinary team in evaluating patient care. Communicates relevant information to promote continuity of care. Is knowledgeable about activities that facilitate collaboration between departments. Participates in the development and achievement of unit goals and performance improvement activities.
