serve.Summary:An Advanced Practice Provider (APP) functions as part
of the interdisciplinary healthcare team in accordance with
privileges approved by the credentialing committee to provide high
quality, cost effective care to patients within the APPs scope of
practice in collaboration with a supervising physician. The APP
reflects the mission, vision, and values of the organization, and
complies with all relevant policies, procedures, guidelines and
other regulatory and accreditation standards.This role will support
the Division of Cognitive Behavioral Neurology to care for patients
with mild to advanced cognitive impairment. Primary
responsibilities will be for management of established patients
receiving continuing care for dementia-related problems, but you
will also participate in clinical triage, quality improvement and
patient-caregiver support initiatives.Responsibilities:1. Clinical-
Our clinic is homed within the Neurology Department and evaluates
and manages patients with neurodegenerative cognitive and
behavioral impairments. Our population is all over 50, and
typically over 65 so comfort with geriatric principles and this
population is a must. You will obtain relevant health and medical
history, perform neurological assessment, review and interpret
pertinent diagnostic tests/results, develop an appropriate
differential diagnose and implement treatment plans including
prescribing/ordering of appropriate evidence-based pharmacological
agents consistent with departmental guidelines and privileging. You
will be responsible for maintaining clear, comprehensive and
structured records within an Epic-based EMR for all aspects of
patient care, and interact extensively with multiple other team
members both verbally and in writing, including neuropsychology,
care navigation, geriatrics and geriatric psychiatry. 2. Education-
In this role, you will have an opportunity to provide education and
support to both the patient and their caregiver(s); to
appropriately and professionally educate clients and/or families
regarding their medical condition, treatment, and follow-up. You
will collaborate with clinicians, staff, and medical learners to
promote continuity and patient/family centered care. You will also
be expected to participate in monthly community/network outreach
programs. Along with the education of others, you will be expected
to actively recognize and participate in identification of
self-learning needs. We provide CE support and encourage attendance
of education programs based on identified learning needs.3. Quality
and Program Initiatives - There are several health-system-wide
initiatives being developed within our Division for care of both
early/mild Alzheimer's disease patients as well as later-stage,
more advanced dementias. You will be engaged in both individual
patient and program-level quality improvement by identifying
limitations and implementing processes for improvement. You will be
asked to collaborate on assessment, problem-solving,
decision-making, and evaluating clinical/system issues and models
of care. When the opportunity arises, you may also participate in
system-level improvements for the clinical enterprise.Other
InformationOther information:Education Requirements:Completion of
an accredited graduate-level Advanced Practice Provider (APRN or
PA) program.Management highly prefers Gerontology Primary Care
track Licensure/Certification Requirements:Licensed as an Advanced
Practice Provider (i.e., Nurse Practitioner, Physician Assistant,
Clinical Pharmacist Practitioner, Certified Nurse Midwife,
Certified Registered Nurse Anesthetist) in the state of North
Carolina. Board certification if required by the Department.
DEA.Professional Experience Requirements:No prior experience
required.Prefer a minimum of 1-year prior experience with post
graduate outpatient Knowledge/Skills/and Abilities
Requirements:Demonstrated clinical competency. Effective leadership
skills. Strong interpersonal skills. Ability to work independently
and collaboratively. Demonstrated written and verbal communication
and strong analytical skills. Effective interaction to ensure the
successful care of patients in the clinical setting. Documentation
in the medical record to substantiate accurate billing.Job
