Expeditor - Experienced RN/Registered Nurse - Patient Flow &
Transfer Center/CAREHub Duke University Hospital - Durham, NC The
Patient Flow team at Duke University Hospital is looking to grow!
An exceptional group of Clinical Professionals dedicated to the
facilitation and coordination of patient movement within a world
class healthcare system, surrounding regional healthcare systems,
and beyond!Department Profile:The Patient Flow team works with
referring agencies, hospitals, clinics, and emergency departments
throughout the state and beyond to coordinate safe and efficient
integration into the Duke University Health System. This team also
facilitates patient movement throughout the facility by
coordinating transfers between the Inpatient Units, PACU, Emergency
Department, and Ambulatory Clinics. This team works alongside
providers to identify patient needs, triage needs based on acuity,
and ensure that patients are relocated in accordance with their
needs.Work Hours & Shifts:
- Day shift available Monday - Friday (8 hour shifts)
- No weekends requiredJob Summary:The Clinical Expediter at Duke
University Hospital plays a pivotal role in optimizing patient flow
and enhancing operational efficiency within the healthcare system.
This position requires a dynamic individual with strong clinical
knowledge and exceptional organizational skills. The Clinical
Expediter collaborates with healthcare teams, including physicians,
nurses, case managers, and support services, to streamline the
discharge process, expedite physician order execution, and
coordinate the timely transfer of patients. They play a key role in
facilitating communication, resolving barriers, and ensuring the
seamless transition of care. The Clinical Expediter actively
identifies opportunities for process improvement, implements best
practices, and maintains a patient-centered approach to enhance the
overall patient experience. By leveraging their clinical expertise
and effective coordination skills, the Clinical Expediter
contributes to optimizing resource utilization, reducing length of
stay, and promoting efficient and effective healthcare delivery at
Duke University Hospital. Responsibilities:
- Demonstrate a deep understanding of CareHub tiles to identify
patterns and implement proactive strategies for patient
progression
- Triage and address barriers to discharge in real-time, freeing
up high-demand beds and optimizing hospital capacity
- Utilize electronic medical records, collaborate with care
teams, and leverage available resources to promptly manage alerts
and prioritize issues
- Escalate clinical priorities and propose strategies for
resolving issues affecting patient progression to CAREHub
leadership and/or CSU leaders
- Collaborate closely with local care teams, ensuring effective
communication and partnership in implementing resolution
pathwaysKnowledge, Skills, Education, and Experience Required:
- Graduation from an accredited Bachelor's Degree in Nursing or
higher
- Minimum of 3 years of RN experience in a medical-surgical
inpatient unit
- Knowledge of Duke Health System hospitals and patient care
units preferred
- Strong communication and problem-solving skills across multiple
platforms and modalities Licensure, Certification or Registration
Required:
- Current registration with North Carolina State Board of Nursing
as a registered professional nurse OR current compact RN licensure
to practice in the state of North Carolina required.
