Duke Homecare & HospiceDuke Homecare & Hospice offers hospice, home health and infusion services as well as serves as the home for the Duke Caregiver Support Program. Our Hospice team works closely with a patient's physician to provide comprehensive, individualized care in the comfort their home or at our inpatient hospice facility located in Durham. Our team of nurses, therapists, social workers, counselors, and chaplains help patients and their families manage advanced medical conditions.Duke Home Health provides services in a patient's home that include skilled nursing, physical therapy, occupational therapy, speech therapy, social service, home health aides, and pain and wound management. Our infusion team comes to a patient's home to administer intravenous medications, including antibiotics and chemotherapy, to children and adults. Our nurses, pharmacists, dietitians, and patient services coordinators work under the direction of a patient's doctor to monitor needs and treatment.Duke Nursing Highlights: -

    • Generous 50-week comprehensive training schedule prepares new nurses with confidence as they rotate through Home Health, Hospice, Home Infusion, and Care Coordination service lines. -
      • Our new nurse resident will be guided and nurtured by an experienced preceptor meeting often with our nurse educator, manager, and program leaders, for additional support and encouragement. -
        • As a successful nurse resident, you will graduate from the PATCH program, and confidently be able to provide evidenced based care and promote positive outcomes for our patients and their loved ones. -
          • Successful nurse residents may choose their preferred work area in either our Home Health, Hospice, or Infusion services. Additional orientation and training will then be provided for their chosen field.Duties and Responsibilities of this Level
            • Plan and provide professional nursing care for patients in accordance with physician orders and established policies and procedures to include collecting and analyzing patient data, developing and implementing nursing care plans, establishing interpersonal relationships with patients, evaluating nursing care given and communicating to proper personnel relevant facts about patients, action taken and patient responses.
            • Provide direction and guidance in the proper use, care and control of material resources; appraise quality and effectiveness of material and equipment and report results to head nurse; maintain standards of cleanliness in assigned areas.
            • Assist in the development and implementation of general policies and procedures designed to affect efficiency of operations.
            • Required Qualifications at this LevelEducation
              • BSN, Associate Degree or Diploma, Nursing required -Experience
                • No experience required
                • Preferred to have 2-3 years of healthcare experience as an LPN, Medical Assistant or Nursing Assistant; community-based experience a plus.Degrees, Licensure, and/or CertificationMust have current or compact RN licensure in the state of North Carolina. BLS required.Knowledge, Skills, and Abilities
                  • Knowledge of scope of the registered nurse, licensed practical nurse and NCA
                  • Knowledge of and appropriate application of the nursing process
                  • Knowledge of professional theory, practice and procedure
                  • Ability to assess nursing needs of acute and chronically ill patients and their families
                  • Ability to independently seek out resources and work collaboratively
                  • Ability to establish and maintain effective working relationships
                  • Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others
                  • Ability to teach patients and families in accordance with the nursing plan of care
                  • Ability to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up
                  • Ability to use fine motor skills
                  • Competent in BLS and/or other specialized life support requirements designated by work area or organization
                  • Ability to record activities, document assessments, plan of care, interventions, evaluation and re-evaluation of patient status
                  • Ability to use computer and learn new software programs
                  • Ability to document and communicate pertinent information using computer and/or paper documentation tools
                  • Ability to navigate the entity to provide clinical care for patients
                  • Ability to withstand prolonged standing and walking
                  • Ability to remain focused and organized
                  • Working knowledge of procedures and techniques involved in administering routine and special treatments to patients
                  • Working knowledge of infection control procedures and safety precautions
