SummaryCase Manager Coordinator is knowledgeable about the program
and is responsible for interacting with the patients, team, and
other stakeholders, both internal and external, assisting the
person served to become oriented to their program, participating
consistently in team conferences concerning the person served,
promotes the participation of the person served on an ongoing basis
with discussions of plans, services, goals, and status throughout
the process, facilitates and ensures the completion of discharge
recommendations including consideration of financial information,
communicates information to the person served, is responsible for
handling all admission inquiries, working in liaison with physician
to ensure appropriate and compliant admissions for the unit,
completing the admission intake process, collecting and entering
the PPS data and transmissions, assisting in the QI process and
data collection, and is responsible for remaining competent and
knowledgeable about CMS and IRF regulations, CMS changes, and
training staff on these changes and regulations.Reports to:
Director of Case Management -Responsibilities of the
PositionCoordination of clinical care (medical necessity,
appropriateness of care and resource utilization for admission,
continued stay, discharge and post- acute care) compared to
evidence-based practice, internal and external requirements. (40%
daily, essential).Identify and report variances in appropriateness
of medical care provided over/under utilization of resources
compared to evidence-based practice and external requirements. This
priority includes work with relevant software and communicating
information through clear, complete, and concise documentation in
eCCM (40% daily, essential).Effective collaboration with
physicians, nurses, ancillary staff, payors, patients, and families
to achieve optimum clinical outcomes (10% daily, essential).Remain
current with relevant clinical/case management practices. (10%
daily, essential).Maintains established departmental policies and
procedures, objectives, performance improvement program, safety,
environmental, and Infection Control standards.Communicates
effectively with customers of all ages.Evaluates learning needs of
patient and/or family and provides patient/family education
appropriate to age, culture, condition, and circumstances.Reports
patient's complaint of pain to the licensed nurse.Works as an
advocate for the physical and emotional well-being of the
patient.Recognizes and meets the needs of customers.Meets
expectations for attendance at Ethics, Forums, meetings, and other
required training/ workshop sessions.Performs other related duties
as assigned and requested.Required EducationAssociate's/Bachelor's
degree in Nursing is required. -Licenses/CertificationsA current
North Carolina or compact state Registered Nurse (RN) license is
required upon hire.Basic Life Support (BLS) through the American
Heart Association (AHA) is required upon hire.Case Management
certification is preferred. -Required SkillsCritical thinking
skills, decisive judgment, and the ability to work with minimal
supervision. -Minimum Work ExperienceRNs with a minimum of 5 years
of bedside experience or LPN with 10 years' experience in the
relevant clinical specialty (e.g., case manager for pediatrics
patients should have pediatric nursing experience) is
preferred.Supervisory experience is preferred.Case Management
